Client Payment Plan

This form allows you to set up a payment plan for your clients should you so desire.  It offers a reasonable monthly payment plan that encourages regular treatment for the client even if they can't afford to pay you at the frequent intervals of their treatment.  They get the treatment they need (but might not otherwise have been able to afford) and you get paid in a reasonable time frame.

It is a written contract that is binding.
